Skilled Care
Rehabilitation and Therapy
We have a dedicated and experienced therapy department specializing in
physical, occupational, and speech therapies. Our team develops an
individualized rehabilitation program to specifically meet the needs of
each resident. Most importantly, we work diligently to help our residents
achieve or exceed their previous level of activity and independence.
In addition, we proudly feature a Medicare unit with all private rooms.
Medicare residents are invited to convalesce in a private room at no
additional charge.
Wound Care
Broadview Health Center’s experienced Wound Care Team is proud of its
continuing success with cases of difficult-to-heal wounds. We are healing
all types of wounds from the simple to the complex in a nursing community
setting. A total skin and wound assessment is conducted on the day of
admission. Low air loss mattresses, protective foot gear, special seating
cushions for wheelchairs, etc. are incorporated on an individual basis of
need. Our dedicated Wound Care Team conducts skin rounds on a weekly basis
to evaluate all wounds with respect to their progress and the efficacy of the
selected treatments. Our Wound Care Coordinator, Elaine LeBlanc, has many
years of nursing experience and is an expert in skin and wound care.
In addition, we have an excellent podiatrist who is active and knowledgeable
in wound care. Dr. Kathleen Graytock is in the facility every two weeks and
more often as needed. She spends a full day seeing residents and evaluating
wounds as well as administering general foot care. All residents are routinely
seen by Dr. Graytock. This eliminates the need for the resident to leave the
facility for podiatry appointments.
Our Therapy department is also very active in our Wound Care program. Therapists
evaluate residents for proper and preventive positioning in bed and chair.
They also evaluate the application of special appliances and treatments from which
residents may benefit.
How do I qualify for Medicare skilled care?
After an individual has had a qualifying hospital stay of three days, that individual
is eligible to be admitted under the Medicare Skilled category. When admitted to a
skilled nursing facility, Medicare will pay 100% of your stay from day 1 through day
20, as long as a qualifying skilled service is being received. From day 21 through
day 100 there is a co-insurance for which the resident or family is responsible.
This charge is often covered by secondary insurance. The amount is dictated by
Medicare and changes yearly. Skilled services include physical, occupational and
speech therapy, wound care, new feeding tubes, IV therapy, and certain breathing
treatments. Please contact the Broadview admissions department for more information.
Discharge Planning
The care and comfort of our residents, whether in the facility or at home, is Broadview’s
chief mission. When a resident is ready to go home, Broadview provides comprehensive
discharge planning. Referrals are made to home health companies for therapy, nursing
issues, home health aides and medical social workers. If any medical equipment or oxygen
is needed in the home, these items are also taken care of before discharge. Referrals are
routinely made to the Senior Options/Passport programs, as well as Adult Day Centers.
Most needs that a resident or family may have can be taken care of before discharge so
that the return home for the resident is as safe and comfortable as possible.
